Consumer Notice

As a client receiving services, you have rights that I want to protect! Here is some important information for you to know:

You are have a right to request your records. These are the steps to take if you would like copies of your records from services I have provided you:

  1. Send me an email to j.cienski@the68collective.com that clearly states what type of records you are requesting (progress notes, treatment summary, invoices, etc.). You need to include the date range you are needing these items to cover, and the reason as to why you are requesting your records. This can also be done in a handwritten format and given to me during session!

  2. I will provide the requested records after verbally confirming your request and ensuring I am reading the request correctly. You will need to choose if you would like to receive your records electronically or physically. Both forms will incur a reasonable fee (unless an lawful exception is in place from the state of Texas) and the fee will be discussed prior to me signing off on the request.

  3. I will then create and invoice that includes the original request, the fee to be collected, and our agreement on date of receipt. I need at least two weeks to procure records, and if the time span covers more than a year, I need three weeks.

  4. Once you have paid the fee, your records will be delivered in a timely manner.
